ST. JUDE, Glorious 
Apostle, faithful servant and friend of Jesus, the name of the traitor has caused you to be    forgotten by many, but the true Church invokes you universally as the Patron of things 
despaired of; pray for me, who am so miserable; pray for me, that finally I may   receive the consolations and the succor of Heaven in all my necessities,  tribulations and sufferings,        particularly (here make your    request), and that I may bless God with the Elect throughout
Eternity.     Amen.

St. Peregrine an angel for his companion, the Mother of God for his Teacher, and Jesus for the Physician of his malady; grant, we 
beseech Thee, through his merits, that we may on earth intensely love our  Savior and the Blessed      Virgin, and in heaven bless them forever.  Grant that we receive the favor for which we now petition, through the same Christ our Lord.   Amen.
